Sliding Fee Program

All patients seeking health care services at the Center of Hope for Behavioral Health are assured that they will be served or connected to resources regardless of the ability to pay. No one is refused access to essential services because of lack of financial means to pay. This program is designed to provide discounted care to those who have limited means to pay for their behavioral healthcare services (uninsured or underinsured).

To determine if the Sliding Fee program may pertain to you, please follow these steps:

  • Using the 2022 Table below, find your family size down the left column
  • Next, find your annual income range on that same line
  • Last, follow the income range column upward to the point that tells you whether you might pay a nominal fee or a designated percentage of the charges.


Note: This is primarily an estimate that should provide enough information to determine whether you want to submit an application or not.
Individuals and families must complete the initial application for approval. The application and all required supporting documents will be reviewed and a decision made regarding the level of adjustment to be assigned. In addition, all participants in the Sliding Fee program must re-apply annually with current proof of income.

What will I need to submit my application?

  • Current Proof of Income: last year’s tax return, paycheck stubs (most recent 2 months required), most recent W-2 Forms, or benefit letters or statements that show proof of what you receive for social security, unemployment, workers compensation, child support, etc. 
  • Identification/Address: Driver’s License, Utility Bill, employment ID, State Identification, or college ID  
  • Fully completed Application, including family size and all information required.

Center of Hope for Behavioral Health will base program eligibility on a person’s ability to pay and will not discriminate on the basis of an individual’s race, color, sex, national origin, disability, religion, age, sexual orientation, or gender identity. The Federal Poverty Guidelines, household size, and income are used in creating and annually updating the sliding fee schedule (SFS) to determine eligibility. The Center of Hope for Behavioral Health Board of Directors may also amend it.
