Sliding Fee Program

All patients seeking health care services at the Center of Hope for Behavioral Health are assured that they will be served or connected to resources regardless of the ability to pay. This program is designed to provide free and discounted care to uninsured people with limited means to pay for their behavioral healthcare services.

To determine if the Sliding Fee program may pertain to you, please follow these steps:

Using the 2025 Table below:

  1. Find your family size down the left column
  2. Next, find your household income range on that same line
  3. Are you uninsured? You cannot have any behavioral health insurance to qualify.

What will I need to submit my application?

ONE Proof of income

  • Bank Statement 1 month of direct deposits
  • federal income tax return/ records,
  • paycheck stubs (2 months required)
  • benefit letters or statements that show proof of what you receive for social security, unemployment, worker’s compensation, child support.

 ONE Proof Tennessee Residency:

  • Valid (not expired) TN Drivers License or state issued Picture IDcard,
  • Voters Card
  • Utility Bill (3 months in applicant name)
  • Bank statement with residence,
  • A signed and dated statement from a person familiar who can verify circumstances (case manager, family member, friend, or employer)

ONE Proof of No Behavioral Health Coverage:

    • Tenncare denial,
    • tn.gov application
    • letter from insurance, no behavioral health or exhaustion of benefits.

SLIDING FEE CHART

Center of Hope for Behavioral Health will base program eligibility on a person’s ability to pay and will not discriminate based on race, color, sex, national origin, disability, religion, age, sexual orientation, or gender identity. The Federal Poverty Guidelines, household size, and income are used to create and annually update the sliding fee schedule (SFS) to determine eligibility.
